Home health care helps people live independently for as long as possible, given the limits of their medical condition. It covers a wide range of services and can often delay the need for long-term nursing home care.

SUPRA provides medically appropriate service without consideration to race, religion, sex, age, disability, or sexual orientation.

 Eligibility

A physician must determine the need for home care and he/she must order services and certify the following:

• The patient is homebound.

• The patient requires skilled nursing care, physical therapy, occupational therapy, speech therapy or medical social worker.

• The services are medically necessary.

• Care can be given effectively on an intermittent basis. 

The Process

Once a doctor assigns you to home care services, the process is as follows:

1. A SUPRA nurse will visit your house to assess your needs.
2. A plan of care will be developed.
3. You will be involved in all the decision making and directing of your care.
4. Scheduled visits with nurses, aides, and therapists will begin.

This will all be designed with you and your family to get you back on the road to recovery.
